Estimating fat without weighing anyone

The body adiposity index, published by Bergman and colleagues in 2011, estimates body fat percentage from hip circumference and height alone. It uses no weight at all, which was the novelty.

The formula is hip circumference in centimetres divided by height in metres raised to the power 1.5, minus 18.

The motivation was practical: in settings without reliable scales, a tape measure is enough. It was developed and validated in Mexican American and African American populations against DEXA measurements.

How well it performed afterwards

Subsequent validation in other populations has been mixed and generally less favourable than the original study. Several studies found it no better than BMI at estimating body fat, and some found it worse.

It tends to overestimate body fat in men and underestimate it in women, and it performs poorly in lean and in very obese individuals.

Which places it as an interesting idea that did not hold up well outside its derivation population. That is a common trajectory for anthropometric indices, and it is why the original publication and the subsequent validation literature should be read together rather than separately.

Measuring the hip properly

The measurement is the maximum circumference around the buttocks, taken with the tape horizontal and the person standing with feet together.

Because the whole estimate rests on this one measurement, technique dominates the result. A centimetre of error in hip circumference shifts the estimated body fat percentage by roughly half a point.

Clothing matters more than for most measurements, since the tape passes over the fullest part of the body. Measuring over light clothing or directly against skin, consistently, is what makes repeated readings comparable.

Where it might still be useful

In field settings without scales, it provides an estimate where the alternative is none. That was the original purpose and it remains the strongest case for it.

It may also be useful as a secondary measure alongside others, since it captures something slightly different from BMI: hip circumference reflects gluteofemoral fat, which has a different metabolic profile from abdominal fat.

For anyone with access to a scale, BMI plus a waist measurement provides more useful information for less effort, and both have far more evidence behind them.

Reading any body fat estimate carefully

Every method of estimating body fat outside a laboratory has a meaningful error bar. Skinfold calipers in skilled hands run around three to four percentage points. Bioimpedance is similar or worse and moves with hydration. Prediction equations from circumferences vary widely.

That error is rarely reported alongside the number, which encourages people to treat a single reading as precise. A result of 24% from any of these methods might reasonably be anything from 20% to 28%.

Which means the trend, measured the same way under the same conditions, carries almost all of the useful signal. A single reading from any method is a rough orientation and nothing more.

The index also illustrates something worth carrying to any new body measure: the population it was derived in matters as much as the formula. This one was built and validated on Mexican American and African American adults, and its weaker performance elsewhere is not a flaw in the arithmetic but a consequence of body proportions differing between populations. Any anthropometric equation carries the population it came from inside it, and applying one outside that population is an extrapolation whether or not it is described as one.
