How should tiers be spaced?

Close enough that the next one feels reachable from the current basket. Doubling the spend for the next tier is usually too far.

How steep should the discounts be?

Steep enough to motivate and shallow enough that contribution per order still rises. Check the contribution at each tier rather than only the discount.

Should the top tier be very generous?

Only if the basket increase justifies it. A dramatic top tier that a handful of customers reach costs little; one that many reach can cost more than it earns.

Does this work better than a flat discount?

Generally yes, because it rewards larger baskets rather than discounting every order equally. A flat discount gives the same away to a £20 order and a £200 one.
